omissions of facts in this application are grounds for disqualification from further

consideration. I also understand that this is an application for a 1099 sub-

contractor position and sub-contractors cannot be under the influence of

uncontrolled substances or alcohol at MY DJ’s events and performances. I authorize

MY DJ’s management to contact all references given in this application and to

conduct a review, including obtaining my report and background check from any

authorized reporting agency.
